間接話法のit wasは形式的には過去だが、意味的には現在（正確に言えば、
主動詞と同時） したがって「今は違う」というニュアンスにはならない。

間接話法に転換すると、
I was told to put on a helmet whenever I entered the(that) area.
後半も同様、まず直説法で考える。「もし誤ってプラットフォームから落ち怪我をすると、
会社にとって大きな問題となるだろう」
条件も帰結も蓋然性がある（反実仮想ではない）ので、
If I fall off from a plafform and get injured, it will cause a big problem for the compay.
主節を過去にして従属節に書き換えると、
I feared that if I fell off from a plafform and got injured, it would cause a big problem
for the compay. がいちばん自然。 would have caused は仮定法過去完了の反実
仮想の帰結節のように読まれるので避けたほうがよいだろう。
なお、従属節で時制の一致を受けないで現在形が可能なのは「誰でもそれとわかる
ような真理」 The teacher told us that the earth orbits the sun in 365 days.
この場合、the earth ORBITEDとか、one and one WAS two のように過去形にすると
不自然。「安全規則を守るべきだ」は形式的に過去形にすることさえ違和感がある
ような「普遍的真理」ではない。
